A design for an integrated version of a charting tool I built to improve documentation in the clinic.
Problem
Physical therapists spend hours per week charting, and do an imperfect job of it. This is costly to the clinic, practitioner and patient care.
Solution
This tool listens to the conversation, synthesizes information into the appropriate categories, and keeps a working diagnosis (which the therapist can modify as shown in the prototype). The therapist only has to modify the relevant parameters rather than creating a chart from scratch. Over time, the tool will learn the individual therapists documentation style and modify itself to anticipate their needs.
